Proposed resolution:

In [facet.num.put.virtuals], paragraph 19, in the Effects: clause for member put(...., bool), replace the initialization of the string_type value s as follows:

const numpunct& np = use_facet<numpunct<charT> >(loc);
string_type s = val ? np.truename() : np.falsename(); 

In paragraph 19, Effects:, members truename() and falsename are used from facet ctype<charT>, but it has no such members. Note that this is also a problem in 22.2.2.1.2, addressed in (4).
