Proposed resolution:
In [locale.category], paragraph 2, change "<cctype>" to read "<clocale>".

Paragraph 2 implies that the C macros LC_CTYPE etc. are defined in <cctype>, where they are in fact defined elsewhere to appear in <clocale>.
